Our History
ChapCare has been putting patients first since 1998. For 25 years, ChapCare has served the communities of the San Gabriel Valley, providing quality healthcare to men, women and children of all ages.
Since 2013, ChapCare has been recognized as a Patient Centered Medical Home (PCMH). A PCMH is a team-based model of care led by a physician who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes.
Our Mission
ChapCare’s mission is to provide excellent, comprehensive, and innovative healthcare that is accessible to all the residents of the San Gabriel Valley.
ChapCare was founded in 1995 to establish primary healthcare services for the low-income, uninsured residents in the Pasadena area.
Today, ChapCare is a network of federally-funded community health centers that provides more than 68,000 primary healthcare visits annually.

Who We Serve
ChapCare serves all the residents of the San Gabriel Valley. We contract with a number of qualified health plans to provide primary healthcare services for those with health insurance.
ChapCare also serves as the community safety net by providing comprehensive healthcare to low income and uninsured individuals and families.
