ChapCare’s Referrals Department works with 211 LA County to connect patients with medical and social services in Los Angeles County. 211 LA County is the largest information & referral service in the nation.

ChapCare is an active member of the California Primary Care Association.
Community Clinic Association of Los Angeles County
ChapCare is an active member of the Community Clinic Association of Los Angeles County. ChapCare’s Senior Staff attends monthly roundtable meetings.
Health Consortium of Greater San Gabriel Valley
ChapCare is an active member of the Health Consortium of the Greater San Gabriel Valley.

Covered California
Covered California provides ChapCare funding through its Navigator Grant Program to expand health insurance outreach, education, enrollment, and post-enrollment assistance in our service area.
Delta Dental Community Care Foundation
Delta Dental Community Care Foundation provided ChapCare with funding to purchase 4 delivery systems and 2 intraoral cameras for our Fair Oaks dental clinic.
Direct Relief
Direct Relief supports general operations of our COVID-19 response through grant funding.
Goodwin Family Memorial Trust
The Goodwin Family Memorial Trust provides grant funding to be used to treat male pediatric patients.
Green Foundation
The Green Foundation provided funding to support the provision of primary healthcare services and COVID-19 response services.
Kaiser Permanente Southern California Community Benefit Program
Kaiser Permanente provided grants funds to support key capacity building activities.
L.A. Care Health Plan
Through L.A. Care Health Plan’s Community Benefits Program, ChapCare received funds through its Oral Health Initiative (OHI) to hire a new dentist. L.A. Care’s Elevating the Safety Net Provider Recruitment Program (PRP) has funded the recruitment of 2 Physicians to support ChapCare’s robust patient population through personalized and consistent care.
Monrovia Rotary Club
The Rotary Club of Monrovia, California provided ChapCare grant funds to help distribute additional flu vaccinations in the Monrovia area.
Pasadena Community Foundation
Pasadena Community Foundation provided grant funding to purchase 3 dental chairs for our Fair Oaks dental clinic.
Pasadena Rotary Club
The Rotary Club of Pasadena, California provided ChapCare grant funds to help distribute additional flu vaccinations in the Pasadena area.

The Patron Saints Foundation provided funding to support the provision of medical services.

Physicians for a Health California provided funds to support COVID vaccination expansion.

Alinea Medical Imaging
Alinea Imaging provides on-site mammograms (via a Mobile Unit) to ChapCare patients twice per month.
Array Behavioral Care
Array Behavioral Care provides tele-psychiatry services to ChapCare patients one day per week.
County of Los Angeles Department of Health Services
ChapCare refers patients in need of specialty care to LAC+USC Medical Center. ChapCare participates in the eConsult program, which is administered by the department.
Foothill Unity Center
Foothill Unity Center provides access to healthy food through its food bank to patients referred by ChapCare.
Greater El Monte Community Hospital
Coordinate on outreach activities in the El Monte and South El Monte areas to ensure that people are aware of our services.
Health Care LA, IPA (HCLA)
ChapCare is a contracted (Medical Home) clinic partner with Health Care LA, IPA (HCLA). Through our partnership, we are able to care for patients enrolled with government sponsored managed care programs in Medi-Cal, Medicare Advantage, and Medi-Medi (dual eligibles). HCLA is the largest IPA network in Los Angeles County that contracts with FQHCs and community health centers.
Huntington Hospital
Huntington Hospital is the preferred hospital for many of ChapCare’s Pasadena patients to deliver their baby. ChapCare refers patients to health education classes administered at Huntington Hospital.
Kaiser Permanente
Kaiser Permanente – Baldwin Park works with ChapCare to coordinate care for in-need community members.
L.A. CADA
L.A. CADA provides on-site substance use counseling services at ChapCare's Fair Oaks health center.
OCHIN
ChapCare participates in OCHIN, one of the nation’s largest health information networks, which brings together hospitals, safety-net clinics, county health departments, university’s, etc. from across the country, allowing providers to pool resources, share best practices, and have access to a broad range of cutting edge health information technology.
Pasadena Public Health Department (PPHD)
At its Pasadena Health Centers, ChapCare is a part of PPHD’s “Vaccines for Children” program. The “Vaccines for Children” program provides vaccines for low-income children.
San Gabriel Valley Consortium on Homelessness
ChapCare is an active participant of the San Gabriel Valley Consortium on Homelessness. The consortium’s mission is to develop and support coordinated, region-wide strategies to alleviate and ultimately eradicate homelessness in the San Gabriel Valley.
Union Station Homeless Services
ChapCare provides homeless outreach services one half day per week at Union Station's Centennial Place.
Western University of Health Sciences
Residents from Western University of Health Sciences in Pomona provide optometry services at ChapCare’s Lincoln health center under the supervision of a university professor (optometrist).
