The CHAP/Fair Oaks Health Center is located at 1855 N. Fair Oaks Ave.  Patients should register in Suite 120 before going to the medical or dental clinics.

The CHAP/Del Mar Health Center is located at 3160 E. Del Mar Blvd. between San Gabriel Blvd. and Madre St.

Our staff are committed to your health and well being.

If you need X-rays, our radiology suite is ready to help.

CHAP’s dental clinic is located on the Ground Floor of our building, providing cleaning and restorative treatment.
