CHAP offers a full range of primary medical care services:

• Physical examinations for people of all ages
• Cholesterol checks
• Cancer screening
• Communicable disease screening
• Vision and hearing testing
• Family planning
• Blood pressure screening
• Lab and radiology
• Arrangements for emergency services
• Medications and Prescriptions
• Case management services
  • assist clients in gaining access to other health and social services
  • referrals to other providers
  • transportation
  • interpretive services for languages not available on staff
  • education about access and appropriate use of services
  • health education and lifestyle review

Medical Services are available:

• Monday-Thursday 8:00am – 7:00pm
• Friday 8:00am – 4:00pm
• Saturday 8:00am – 1:00pm
